Tess arrives at a rental house in a gutted Detroit neighborhood for a job interview and finds it already occupied by a polite man named Keith. With no hotels available in the area she agrees to stay. Keith seems genuinely harmless. In the basement, Tess discovers a hidden door opening onto a dank tunnel with a mattress, a camcorder, and a water bottle — signs someone was kept there. Keith explores further and is killed by something massive in the darkness. In a parallel storyline, sleazy TV actor AJ drives to Detroit after assault accusations derail his career — he owns the property through a shell company and goes to sell it. He finds Tess's car, investigates the tunnels, and encounters the Mother: a massively strong, pale, feral human-like creature. A third timeline reveals the tunnels were built by a man named Frank who used them to rape and imprison women, breeding children in total isolation for decades. The Mother is a product of extreme generational inbreeding. AJ pushes Tess off a water tower to distract the Mother and escape; she survives. AJ is brought back into the tunnels and killed. Tess lures the Mother out of the building and shoots her dead.
